Neighborhood Local Food Survey

Dear East Side Hill Resident,
Thank you for your willingness to take this survey. It is intended to help us better understand local food practices and perspectives here in Eau Claire. The survey is being conducted in conjunction with the students of my geography of food class at UWEC and my ongoing research interests. I am also an East Side Hill resident since 2002. In addition to the web survey, some of my students will also be walking door-to-door in the neighborhood to conduct the survey face-to-face.
Completing and returning the survey will be taken as your consent to participate.

As explained in the cover letter introducing this research, you should know the following before you start:

Your answers are anonymous;

You may skip any question or choose to exit the survey at any time;

At the end of the survey, you will have the opportunity to identify yourself, but that is entirely voluntary and will only be used for follow-up or to ask more detailed questions;

The survey should take 15-20 minutes to complete. It has 36 questions, most of which ask you to choose from a set of options;

The results of the survey will be posted on the East Side Hill Neighborhood website.
